Appointment Information

First Visit
Parents of each new patient are asked to complete a health history form before their first visit to Piedmont Pediatrics. This will help our medical staff make a prompt and accurate assessment of your child during the first visit. Please bring your current insurance cards, including Medicaid cards, and any current prescription bottles. Please arrive 20 minutes before your scheduled appointment to complete registration and other paperwork, or print and complete the online forms before your arrival.

Routine Visit
The goal of Piedmont Pediatrics is to provide children with prompt, easy access to quality healthcare. Our medical staff is available to see patients by appointment during normal office hours. Please arrive 15 minutes before your scheduled appointment.

Always bring your insurance cards, including Medicaid cards and all prescription medications.

Cancellation Policy
Please call to cancel any appointments you will be unable to keep at least 24 hours in advance so we may offer these appointments to other patients in need of treatment.
